(1) These rules may bs called the Environment (Protection) Amendment Ruks , 1993 . They shall come into force on the date of their publication in the Official Gazette . 2 . In the Environment (Protection) Rules , 1986,— (a) in rule 14,— (i) for the words "audit report " wherever they occur , the word "statement " shall be substituted; (ii) for the figures, letters and words "15th day of May " the words "thirtieth day of September, " shall be sub - stituted . (b) In Appendix 'A' , for FORM-V , the following form shall be substituted , namely :— "FORM-V (Sec rule 14) Environmental Statement for the financial year ending the 31st March PART—A (i) Name and address of the owner/occupier of the industry operation or process . "' (ii) Industry category Primary.-~(STC Code) Secondary.-~(SIC Code) (iii) Production capacity.—Units— (iv) Year of establishment , (v) Date of the last environmental statement submitted .
